About the role
Key responsibilities & impact- Support the case management process to facilitate and maintain improved healthcare outcomes for members
- Provide advocacy, collaboration, coordination, support, and education using care management tools and resources
- Evaluate members through care management assessments and information/data review
- Recommend approaches to resolve care needs by evaluating benefit plans and internal/external programs and services
- Identify high-risk factors and service needs and implement early, proactive support interventions
- Coordinate and implement Wellness care plan activities and monitor member care needs
- Consult with case managers, supervisors, Medical Directors, and other health programs to overcome barriers
- Present cases at case conferences for multidisciplinary review
- Identify and escalate quality-of-care issues through established channels
- Negotiate appropriate options and services to meet members’ benefit and healthcare needs
- Use motivational interviewing and influencing skills to engage members and promote lifestyle and behavior changes
- Provide coaching, information, and support to empower independent medical and healthy lifestyle choices
- Help members participate knowledgeably with providers in healthcare decision-making
- Monitor, evaluate, and document care in compliance with regulatory, accreditation, and company requirements
- Travel regionally within Kentucky up to 50% of the time using a personal vehicle
- Travel to the Louisville office for meetings and training
Requirements
What you’ll need- Experience and detailed knowledge of Foster Care and juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services, and evidence-based practices applicable to Kentucky SKY populations
- 2 years’ experience in behavioral health, social services or an appropriate related field equivalent to program focus
- Computer literacy and proficiency with internal/external computer systems and MS Office Suite, including Word and Excel
- Flexibility to work beyond Monday-Friday, 8am-5pm core business hours
- Knowledge of growth and developmental milestones
- Case management experience
- Discharge planning experience
- Managed Care experience
- Minimum of a Bachelor's degree or a non-licensed master level clinician degree in behavioral health or a relevant human services field, such as social work, psychology, marriage and family therapy, or counseling
- Valid KY driver's license
- Proof of vehicle insurance and reliable transportation
- Residence in a county in the assigned Salt River Trail region
- Direct/hardwired internet connection within 7 feet of the computer, with minimum 25 Mbps download and 3 Mbps upload speeds; WiFi and satellite internet are not permitted
- Quiet, secure, private, distraction-free home work location compliant with CVS Health and HIPAA guidelines
- Must work within the state and city where currently residing
- Ability to provide own workspace furniture
- Ability to use multiple monitors and applications simultaneously
- Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
- Experience with Microsoft Office 365 applications or similar Google Workspace tools
- Ability to access secure systems such as VPN and EHR portals, maintain computer and password security, and recognize suspicious emails or links
- Ability to troubleshoot common technical issues and contact IT when needed
- Openness to learning new skills
