About the role

Key responsibilities & impact

Conduct face-to-face assessments with members
Evaluate members' medical needs
Facilitate members' overall wellness and help them obtain needed services
Address requests for adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program, and MLTSS program enrollment
Complete a minimum of 15-20 assessments per week, dependent on need
Gather information from various sources to address conditions, including co-morbidities and multiple diagnoses impacting functionality
Assess members' work capacity and related restrictions or limitations
Travel in the field across Mercer and Middlesex County, New Jersey

Requirements

What you’ll need

Must hold an active unrestricted Registered Nurse license in the state of New Jersey
Must possess reliable transportation
Willing and able to travel up to 85% of the time
2+ years clinical practice experience in a hospital, alternative care, home health, or ambulatory care setting
Successful completion of the NJ Choice certification is a contingency of employment
NJ Choice exam score of 80% or higher, successful completion of State training modules, and field mentoring component required
Must be able to work Monday - Friday, 8:00am - 5:00pm EST
Experience using personal computer, keyboard navigation, multiple systems and applications
Experience using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.)
Minimum Associates Degree in Nursing required
Assessment, Case management, Managed Care and/or discharge planning experience preferred
Crisis intervention experience preferred
Clinical interviewing preferred
Bachelor's in Science and Nursing preferred
