Registered Nurse, Field Assessor

About the role
Key responsibilities & impact- Conduct face-to-face assessments with members to evaluate medical needs and overall wellness
- Help members access Adult & Pediatric Medical Daycare, Personal Care Assistance, Nursing Facility Custodial Care, Personal Preference Program, and MLTSS Program Enrollment
- Complete 20+ assessments per week based on need using a holistic approach
- Consider co-morbidities, multiple diagnoses, and functional limitations during assessments
- Collaborate with the team and supervisor to overcome barriers and achieve goals
- Interview members to engage them and uncover their health needs
- Mentor new team members after mastering the role
- Operate at full capacity by month four following training and support
- Travel throughout assigned New Jersey counties
Requirements
What you’ll need- Must reside in Passaic or Morris County, New Jersey
- Caseload and travel spread across Passaic, Morris, Sussex, and Warren Counties
- Reliable transportation and willingness and ability to travel up to 85% of the time
- Active, unrestricted New Jersey RN license
- 1 or more years of clinical experience in hospital, home health, or ambulatory care
- Comfortable working with diverse populations in various settings
- Strong attention to detail and organizational skills
- Ability to work independently
- Computer proficiency and experience handling medical records
- Successful completion of NJ Choice Certification, including minimum exam score of 80%, state training modules, and field mentoring component
- Ability to work Monday–Friday, 8:00 AM–5:00 PM EST
- Assessment, case management, or discharge planning experience preferred
- Managed care or crisis intervention background preferred
- Strong interviewing and communication skills preferred
- Proficiency with MS Office Suite and EMR systems preferred
- Associate Degree in Nursing required; BSN preferred
