Key responsibilities & impact

Work with assigned Aetna Medicare Provider Collaboration groups to improve performance
Lead clinical performance improvement projects and manage multiple complex deliverables through implementation
Support population health initiatives primarily impacting Stars and quality, as well as utilization and cost
Establish trusting relationships with provider partners
Serve as clinical expert on opportunities related to case management, gap closure, and workflow discussions
Partner with case manager supervisors/managers and case managers
Serve as internal and external liaison and clinical resource
Interpret data and educate stakeholders on using data for performance improvement
Strategize with subject matter experts and clinical teams to drive outcomes and improve efficiencies
Coordinate and conduct clinically focused meetings to identify opportunities and prioritize goals and interventions
Collaborate across multiple functions and segments to obtain agreement from impacted parties
Collaborate with Engagement Managers, Local Market Team, Data Team, Medical Operations, and others on Stars/quality improvement initiatives
Travel up to 25% may be required in support of Value Based Care groups
Work Monday through Friday, 8 a.m. to 5 p.m., with no nights, weekends, or holidays

What you’ll need

Registered Nurse with active and unrestricted license in the state the candidate resides
Reside in the Northwest/Mountain area: Idaho, Montana, Nevada, Oregon, Utah, Washington, or Wyoming
4+ years of case management experience
3+ years of clinical experience
1+ year of leadership or supervisory experience
4+ years of Outlook experience
4+ years of Microsoft PowerPoint experience
4+ years of Microsoft Excel experience
1+ years of SharePoint experience
Strong communication and interpersonal skills
Proven track record in meeting project milestones and negotiating for resources
Proven negotiation skills
Proven project management skills
Proven success while working in a matrixed environment
1+ year of managed care experience preferred
Registered Nurse / Bachelor's Degree preferred or equivalent experience
