About the role

Key responsibilities & impact

Partner with a registered nurse case manager in a collaborative process of assessment, planning, and advocacy
Meet members’ benefit plan and/or health needs
Research available options and resources for members with financial, transportation, safety, and housing needs
Provide telephonic and electronic member contact across the entire lifecycle population
Empower members to make positive lifestyle and medical choices
Collaborate with clinical staff to assist members with medical, financial, and behavioral health community resources
Work Monday-Friday 8am-4:30pm EST
Provide no weekend, holiday, or travel coverage

Requirements

What you’ll need

Must live in North Carolina
Master's Degree in Social Work
Active, unrestricted licensure as one of the following: LSW (Licensed Social Worker), LMSW (Licensed Master Social Worker), LCSW (Licensed Clinical Social Worker), LISW (Licensed Independent Social Worker)
2+ years of experience in medical social work or case management
Ability to obtain and maintain multi-state licensure (minimum of 10 states – willing to obtain additional licenses) before completing new hire training
Must be able to work EST hours
Monday-Friday 8am-4:30pm EST availability
