About the role

Key responsibilities & impact

Deliver remote patient care through a virtual technology platform
Independently provide holistic, evidence-based assessment, diagnosis, treatment, health-problem management, health counseling, and disposition planning
Provide pregnancy prevention, STI prevention, safer-sex, contraceptive-care counseling, and medication management
Evaluate primary care, acute, chronically ill, and transitional-care patients aged 18 months and above
Educate patients on health maintenance and respond to patient-care inquiries
Document patient care in the EHR according to MinuteClinic policies and procedures
Coordinate care with internal and external colleagues, including the broader patient-centered medical home
Work within a patient-care team with Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members
Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

Requirements

What you’ll need

Current, unrestricted license in any US state/district
Willingness and ability to obtain additional licensure as needed
Minimum of two years of medically-relevant experience or equivalent
Effective verbal, written, and electronic communication skills
Outstanding organizational skills and ability to multi-task
Initiative, problem solving ability, adaptability and flexibility
Ability to work without direct supervision and practice autonomously
Proficiency with information management and technology
Capacity to collaborate with professional colleagues
Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required
Completion of a Master’s Degree level Family Nurse Practitioner program
Current National Board Certification
State of Employment license to practice in the Advanced Practice Nurse role
Broadband connectivity
Quiet setting with a neutral background for remote visits
Ability to uphold patient privacy per CVSH guidelines
Ability to work 20+ hours per week, including some weekends
