About the role

Key responsibilities & impact
  • Manage Home Hemodialysis patients
  • Educate patients and caregivers in-clinic so they can perform dialysis independently at home
  • Train patients and caregivers on HHD procedures and self-care
  • Manage a caseload of HHD patients, including assessments and care planning
  • Monitor treatment progress
  • Conduct home visits
  • Provide on-call support
  • Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
  • Manage an independent caseload with Monday–Friday, 8-hour shifts

Requirements

What you’ll need
  • RN license in the state of practice
  • Valid driver’s license, insurance, and reliable transportation
  • ADN required; BSN preferred
  • Basic computer skills, including MS Word and Outlook
  • Dialysis experience preferred, including HHD, PD, chronic, or acute dialysis
  • Patient education or teaching background preferred
  • Case management or telehealth experience preferred
  • CNN or CDN certification preferred
