About the role
Key responsibilities & impact- Train patients and caregivers on Home Hemodialysis procedures and self-care
- Manage a caseload of Home Hemodialysis patients
- Conduct patient assessments and care planning
- Monitor treatment progress
- Conduct limited home visits
- Provide on-call support
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
- Educate patients and caregivers in the clinic to enable independent home dialysis
Requirements
What you’ll need- RN license in the state of practice
- Valid driver’s license
- Valid auto insurance
- Reliable transportation
- ADN required
- Basic computer skills, including MS Word and Outlook
- Dialysis experience preferred, including HHD, PD, chronic, or acute dialysis
- Patient education or teaching background preferred
- Case management or telehealth experience preferred
- CNN or CDN certification preferred
