About the role

Key responsibilities & impact

Function autonomously and collaborate with all members of the healthcare team
Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
Minimize fragmentation of the healthcare delivery system
Coordinate care between patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager
Provide telephonic-based, field-based, or combined telephonic and field case management
Support integrated care management for patients with kidney disease throughout the United States
Help prevent complications, reduce avoidable hospitalizations, and improve overall health
Manage clinical, financial, and patient satisfaction outcomes
Accommodate patient outreach across nationally covered patients

Requirements

What you’ll need

A.D.N degree from an accredited school of nursing required; B.S.N preferred
Current Registered Nurse (RN) license in the state of practice required
Minimum of 1 year of IKC RN experience required
Ability to work Monday-Friday
Ability to work autonomously
Continuing education credits maintained as required by state of practice required
Minimum of three (3) years’ experience in clinical nursing required
Minimum of one (1) year’s experience in renal nursing or case management preferred
Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
Demonstrated experience and effectiveness in change agent role
Demonstrated knowledge and understanding of CQI techniques
Current CPR certification required
Ability to adapt to new/electronic processes and increased productivity expectations
Basic computer skills and proficiency in MS Word and Outlook required
Functional proficiency with DaVita-specific clinical software programs, including Capella, required within 90 days of employment
Home office with internet connectivity of at least 1MB upload and 1MB download speed required
Willingness to adjust work hours as needed to accommodate patient outreach
Compact License preferred
MST or PST preferred
