About the role

Key responsibilities & impact
  • Function autonomously and collaborate with healthcare team members to coordinate and facilitate quality, cost-effective care
  • Minimize fragmentation of healthcare delivery for ESRD and CKD patients
  • Coordinate care among patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager
  • Travel 3–4 days per week within an assigned territory
  • Accommodate patient needs and nephrology partner availability on a Monday–Friday schedule
  • Provide comprehensive care and complex chronic condition management for kidney disease patients
  • Support patients in early-stage kidney disease, transitioning to dialysis, seeking kidney transplant, or receiving dialysis

Requirements

What you’ll need
  • A.D.N degree from an accredited school of nursing required; B.S.N preferred
  • Three-year diploma from an accredited diploma program may be substituted for nursing degree
  • Current Registered Nurse (RN) license in the state of practice required
  • Continuing education credits maintained as required by state of practice required
  • Minimum of five (5) years’ experience in clinical nursing strongly preferred
  • Minimum of three (3) years’ experience in renal nursing preferred
  • Case management experience required; alternatively, 3 or more years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions
  • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
  • Knowledge of data and managing clinical, financial, and patient satisfaction outcomes
  • Experience and effectiveness in a change agent role
  • Knowledge of CQI techniques
  • Previous healthcare performance coaching experience required
  • Current CPR certification required
  • Ability to adapt to new/electronic processes and increased productivity expectations related to Capella implementation
  • Current driver’s license in the state employed, positive driving record, and ability to meet insurance coverage requirements
  • Basic computer skills and proficiency in MS Word and Outlook required
  • Functional proficiency with DaVita clinical software programs, including Capella, required within 90 days of employment
  • Home office with internet connectivity of at least 1MB upload and 1MB download speed required
