About the role

Key responsibilities & impact
  • Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
  • Minimize fragmentation of the healthcare delivery system
  • Coordinate care among patients, families, physicians, providers, care teams, communities, and the Regional Operations Manager
  • Work autonomously and collaboratively with the healthcare team
  • Travel 3–4 days per week within an assigned territory
  • Provide coverage for Bel Air, Maryland, and surrounding areas
  • Accommodate patient needs and nephrology partner availability
  • Manage complex chronic conditions and transitions to dialysis, kidney transplant, or ongoing dialysis care
  • Support value-based, person-centered integrated kidney care and patient outcomes

Requirements

What you’ll need
  • ADN degree from an accredited school of nursing required; BSN preferred
  • Three-year diploma from an accredited diploma program may substitute for nursing degree
  • Current Registered Nurse (RN) license in the state of practice required
  • Continuing education credits maintained as required by state of practice required
  • Minimum of five years’ experience in clinical nursing strongly preferred
  • Minimum of three years’ experience in renal nursing preferred
  • Case management experience required, or 3+ years assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions
  • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
  • Knowledge of data and managing clinical, financial, and patient satisfaction outcomes
  • Experience as a change agent
  • Knowledge of CQI techniques
  • Previous healthcare performance coaching experience required
  • Current CPR certification required
  • Ability to adapt to new/electronic processes and increased productivity expectations for Capella implementation
  • Current driver’s license in the state employed, positive driving record, and ability to meet insurance coverage requirements
  • Basic computer skills and proficiency in MS Word and Outlook required
  • Functional proficiency with DaVita clinical software programs, including Capella, required within 90 days of employment
  • Home office with internet connectivity of at least 1MB upload and 1MB download speed required
