About the role

Key responsibilities & impact
  • Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
  • Minimize fragmentation of the healthcare delivery system
  • Function autonomously and collaborate with the healthcare team
  • Coordinate care among patients, families, physicians, providers, care teams, communities, and the Regional Operations Manager
  • Support ESKD and CKD patients throughout California, Oregon, Washington, and Nevada as needed
  • Adapt territory assignments based on business needs
  • Provide remote float case management coverage Monday–Friday
  • Accommodate patient needs and nephrology partner availability

Requirements

What you’ll need
  • A.D.N degree from accredited school of nursing required; B.S.N preferred; three-year diploma from accredited diploma program may be substituted for nursing degree
  • Current Registered Nurse (RN) license in the state of practice required, with licensure in California, Oregon, Nevada, and Washington required or willingness to obtain any missing state license
  • Continuing education credits maintained as required by state of practice required
  • Case management experience required, or 3 or more years of experience assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions
  • Minimum of five years’ clinical nursing experience strongly preferred
  • Minimum of three years’ renal nursing experience preferred
  • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
  • Knowledge of data and managing clinical, financial, and patient satisfaction outcomes
  • Experience and effectiveness in a change agent role
  • Knowledge of CQI techniques
  • Previous healthcare performance coaching experience required
  • Current CPR certification required
  • Ability to adapt to new/electronic processes and increased productivity expectations for Capella implementation
  • Current driver’s license in state employed, positive driving record, and ability to meet insurance coverage requirements
  • Basic computer skills and proficiency in MS Word and Outlook required
  • Functional proficiency with DaVita clinical software programs, including Capella, required within 90 days of employment
  • Home office with internet connectivity providing at least 1MB upload and 1MB download speed
