About the role
Key responsibilities & impact- Cover three clinics in Elmore, Prattville, and Opelika
- Work typical hours of 8:00–4:00, Monday through Friday
- Serve as a vital member of each patient’s core care team
- Support and advocate for patients transitioning to and living with dialysis
- Help patients understand their rights and responsibilities
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease
- Build meaningful, long-term relationships with patients and their families in an intimate outpatient setting
- Collaborate with and rely on the care team in a positive environment
- Commit to improving patient health through clinical goal-setting and quality improvement initiatives
Requirements
What you’ll need- Meets all state required regulations to practice in the dialysis setting
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- LMSW required in the state of Alabama
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook
- Functional proficiency with DaVita specific applications within 60 days
