About the role
Key responsibilities & impact- Work in a team structure with the RN Case Manager and other Integrated Care department staff to coordinate transitions of care and complex discharge planning
- Conduct psychosocial assessments and use intervention strategies to help patients and families understand and cope with illness and disability
- Assist families in resolving complex personal and environmental difficulties affecting social functioning and access to medical care
- Participate in patient care planning with hospital and community agency personnel
- Provide education and consultation to healthcare professionals and the community regarding psychosocial factors
- Deliver organized and integrated social work services to patients, families, and significant others
- Collaborate interdepartmentally with inpatient, ambulatory, and Emergency divisions
- Support onboarding, training, and job shadowing for newer staff and students
- Participate in the department’s peer mentoring program
- Develop social work skills and complete additional special projects as requested
Requirements
What you’ll need- Masters Degree in Social Work from an accredited institution required
- 1-2 years working within Healthcare or Hospital preferred
- Licensed through the State of Ohio Counselor, Social Worker and Marriage and Family Therapist Board, LSW required
- Professional skills in operating technology and providing customer service
