About the role

Key responsibilities & impact
  • Manage the development, implementation, and evaluation of population health and quality strategies
  • Improve access to care, preventive service utilization, health equity, and oral health outcomes for underserved and at-risk populations
  • Analyze quality and utilization data to identify trends, disparities, performance gaps, and target populations
  • Develop and execute outreach plans for Medicaid, Medicare, and other government-sponsored populations
  • Oversee care coordination and quality performance staff, including hiring, coaching, performance management, professional development, and workload prioritization
  • Provide subject matter expertise on population health, oral health education, community engagement, care coordination, preventive health initiatives, and quality measures
  • Develop, implement, and monitor outreach policies, procedures, standards, and best practices
  • Manage budgets, resources, contracts, and outreach program activities
  • Oversee educational materials, presentations, and health promotion campaigns
  • Collaborate with the data analysis team to define and monitor program metrics, evaluate performance, develop reports and recommendations, and identify continuous improvement opportunities
  • Perform other related assigned duties as necessary

Requirements

What you’ll need
  • Bachelor’s degree in dental hygiene, public health, health education, health administration, or a related field
  • Three years of experience in dental hygiene, public health, population health, outreach, care coordination, community health, or related healthcare programs
  • Experience working with clinical quality programs and initiatives
  • Two years of leadership experience
  • Strong knowledge of population health management, preventive healthcare, health equity, and community outreach principles
  • Ability to analyze and interpret data to develop actionable outreach strategies
  • Strong presentation, facilitation, and public speaking skills
  • Strong relationship-building and stakeholder engagement abilities
  • Ability to collaborate effectively across departments and with external partners
  • Strong project management and organizational skills
  • Strong written and verbal communication skills
  • Ability to work independently, manage multiple priorities, and achieve results in a team-oriented environment
  • A quality professional certification, such as CPHQ or HCQM, is preferred
  • Knowledge of dental terminology, dental benefits, procedure coding, and healthcare delivery systems is preferred
