About the role
Key responsibilities & impact- Monitor and manage care coordination initiatives
- Help DWIHN, its provider network, and the healthcare community progress toward a holistic, integrated, broad-ranging service model
- Follow NCQA guidelines for Complex Case Management
- Maintain a caseload of DWIHN members
- Complete required documentation in MHWIN within required time frames
- Visit members in hospital or community settings
- Complete assessments and plans of care within required time frames
- Identify eligible members through hospital inpatient lists, EMS lists, PHQ, hospital recidivism, and other sources
- Oversee marketing and education of Wayne County community providers
- Receive and respond to complex case management referrals within designated time frames
- Complete weekly and monthly reports
- Provide transitions-of-care support for members discharged from acute care settings
- Maintain compiled member lists/data for reporting requirements
- Coordinate with member care teams at the community level
- Connect members with community resources and health providers
- Assist members with applications for health benefits, SSI, Bridge Card, and related support
- Participate in care review mock audits and file reviews
- Provide care coordination for members not eligible for Complex Case Management
- Attend weekly team meetings, all-staff meetings, DWIHN meetings, designated CRSP/COPE/Children/Hospital Liaison meetings, and required training
- Perform related duties as assigned
Requirements
What you’ll need- Bachelor’s Degree in Social Work, Psychology, Counseling, Nursing, Human Services, Social Services, Public Health, Public Administration, Healthcare Administration, Health Management, or a related field
- Four (4) years of full-time paid professional experience in a human service, social service, mental health, or behavioral health setting
- Licensed Bachelor’s Social Worker (LBSW) or higher-level license
- Valid State of Michigan Driver’s License with a safe and acceptable driving record
- Knowledge of DWIHN policies, procedures, practices, provider network, and community resources
- Knowledge of the Michigan Mental Health Code, MDHHS policies, and applicable federal policies, rules, regulations, and procedures
- Knowledge of MHWIN, LOCUS model, clinical care processes, adult continuums of care, integrated care services, Medicaid, Medical Necessity Criteria, ASAM, DSM-V, NCQA guidelines, CMHSPs, and PIHPs
- Assessment, evaluation, attention to detail, time management, report writing, critical thinking, interpersonal, communication, active listening, computer, and teamwork skills
- Proficiency with Word, Excel, Access, PowerPoint, Outlook, and Teams
- Ability to communicate orally and in writing and work effectively with diverse populations
- Judgment and reasoning ability
- Proof of being fully vaccinated for COVID-19 as a condition of employment, subject to legally required accommodations or exemptions
