About the role

Key responsibilities & impact
  • Perform initial assessments and develop Palliative Plans of Care for members newly enrolling to the Palliative Care Program
  • Follow a longitudinal panel of the highest risk patients via regular telephonic and/or video encounters
  • Provide consultative visits, including advanced symptom management, complex goals of care, advanced disease education, prognostication, medical orders, and hospice transitions
  • Evaluate hospital admissions for root causes
  • Support transitions of care for engaged patients
  • Identify readmission trends and help develop solutions for achieving or maintaining a readmission rate less than or equal to 15%
  • Participate in regular huddles with clinical and operations team members
  • Participate in weekly interprofessional team meetings
  • Conduct monthly panel reviews with the Medical Director and Operations Manager
  • Support the team during periods of rapid growth or PTO/absences, including phone-line coverage, outreach calls, initial visits, and monitoring incoming consults
  • Collaborate with interdepartmental clinical teams and leadership to coordinate seamless care
  • Attend Devoted Medical meetings as appropriate
  • Provide rotating on-call weekend and holiday coverage
  • Support the Devoted Health mission by caring for members like family

Requirements

What you’ll need
  • Must possess a current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state.
  • Must be willing to obtain additional state licenses as required by the business.
  • 5 years in advanced practice, with at least 3 in Palliative care
  • ACHPN® credential, or ability/willingness to obtain within first six months of employment
  • Willingness to flex between time zones
  • Comfortable with technology and a fast-paced environment
  • Self-starter with ability to succeed in a work-from-home environment
  • Strong interpersonal and communication skills
  • Comfort working within an interprofessional care team
  • ELNEC® or EPEC® training preferred
  • Prior ICU, oncology or geriatrics nursing experience preferred
  • Prior home-based palliative care experience preferred
  • Prior interprofessional team leadership experience preferred
  • Prior program development experience preferred
