About the role
Key responsibilities & impact- Provide expert clinical care for the Palliative Care Program
- Provide direct supervision of palliative care clinical staff
- Provide direct patient care by phone or video visit during scheduled and on-demand visits
- Participate in referral management, process improvement, and change management
- Participate in or lead patient panel reviews
- Participate in weekly interdisciplinary team meetings
- Act as a clinical champion and player-coach modeling clinical excellence
- Support development and implementation of new workflows during rapid iteration cycles
- Supervise palliative care clinical staff, including NPs, RNs, and Social Workers
- Set goals and coach direct reports to achieve leadership-defined business goals
- Review direct reports’ work and hold RNs and Social Workers accountable for performance
- Create performance improvement plans when needed
- Perform regular clinical quality audits, including note review and listening tours
- Steward a member-centric and open clinical culture
- Recruit, interview, hire, and onboard new clinical staff
Requirements
What you’ll need- 5+ years of experience as a Palliative Care Clinician
- 2+ years of experience leading front line clinical teams with accountability for team level outcomes
- Demonstrated ability to champion new initiatives and achieve direct business outcomes
- Great clinical and non-clinical judgment and ability to coach around performance to drive clinical effectiveness
- Deeply empathetic and humanistic approach to patients
- Ability to work well with individuals from varied backgrounds and skill-sets
- Commitment to the mission and ability to work through stressful, fast-paced conditions
- Willingness to learn from every experience and take risks
- Team-oriented, can-do attitude
- Must hold or meet the requirements for the relevant advanced practice nursing professional license/credentials implied by the Palliative Care NP role
