About the role

Key responsibilities & impact

Conduct telephonic and video visits with patients and caregivers
Establish trust and build strong relationships with patients
Collaborate with clinicians, nurses, and care coordinators to ensure cohesive, patient-centered care
Conduct social work case management interventions
Perform psychosocial screenings and create personalized care plans
Provide emotional and caregiver support
Navigate family/caregiver support
Address social determinants of health and social drivers of utilization
Provide behavioral health navigation
Support long-term care planning and advance care planning
Balance multiple competing priorities from patients
Help family members/caregivers understand patient needs and how to support them
Facilitate and participate in joint visits
Participate in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development
Build relationships and coordinate with key local market and internal stakeholders
Document all clinical interactions in the electronic health record

Requirements

What you’ll need

Master’s degree in social work and active license
Minimum of 2 years expertise in the medical field
Comfortable with technology and a fast-paced environment
Self-starter with ability to succeed in a work-from-home environment
Strong interpersonal and communication skills
Comfortable working with patients over video and phone
Bilingual in English and Creole, Spanish, or Vietnamese (nice to have)
Experience in health insurance (Medicare Advantage a plus) (nice to have)
Experience in geriatrics (nice to have)
