About the role
Key responsibilities & impact- Partner with the Market President to build out and manage the entire market team across Arizona
- Lead clinical teams in the market, including Nurse Practitioners and PAs
- Engage in direct patient care with a small panel of high-risk patients
- Provide in-home care in patients’ homes, community centers, dialysis facilities, or hospitals
- Ensure new patients are seen in a timely manner
- Ensure clinical documentation is thorough and accurate
- Develop team members’ problem-solving skills to prevent unnecessary medical utilization
- Provide ongoing education and evaluation of market-specific APP competencies
- Teach diagnostic skills, evidence-based treatment protocols, treatment abilities, and population health management
- Collaborate with the Market President to deliver KPI and overall P&L results
- Upskill APPs through high-intensity provider coaching based on KPI performance
- Manage highest-need, highest-risk patients, then transition them to APP panels as appropriate
- Lead health plan and network provider partnership discussions
- Manage partner relationships to drive growth, engagement, and clinical/financial outcomes
- Support board material content and creation
- Perform other duties as assigned
Requirements
What you’ll need- Must live in Arizona
- M.D. or D.O. Graduate
- Board Certification in Family Medicine or Internal Medicine Required
- Fellowship training in Geriatrics
- Other professional degrees (MPH, MBA, JD) welcome but not required
- 5-15 years experience in outpatient or home-based practices; value based care experience preferred
- Extensive experience in clinical leadership roles coaching and developing NPs, PAs or MDs
- Experience utilizing KPIs and data to drive improved cost, quality and clinical outcomes for patients
- Comfort with ambiguity; experience in a start-up environment is helpful
