About the role

Key responsibilities & impact
  • Support multiple hospitals, each with a daily census of 30–60 patients
  • Use ACEIT and medication processes to assess, plan, intervene when necessary, implement, and evaluate patient care
  • Obtain census and confirmed admissions from the Virtual Admission Board for each facility
  • Prepare basic documents and analysis using Microsoft Word and Excel
  • Access clinical systems for all hospitals serviced through remote connectivity
  • Enter, verify, and manage medication orders in Powerchart and PharmNet
  • Review drug interaction, allergy, duplication, overdose-range, and renal-dosing alerts and resolve issues appropriately
  • Document clinical interventions, including renal dosing, vancomycin dosing, and medication issues
  • Communicate resolved and ongoing issues to hospital-based pharmacists according to reporting requirements
  • Recommend cost-effective formulary medications and convert TNF orders to valid product orders when possible
  • Manage protected health information according to privacy rules
  • Calculate pharmacokinetic doses and intervals, review creatinine clearance, and recommend renal dosing
  • Apply hospital-specific policies and procedures during medication documentation and verification
  • Evaluate laboratory values for medication therapy appropriateness
  • Accurately document transfer, discharge, home, and ER medication lists in Cerner and alert the verification pharmacist to problems

Requirements

What you’ll need
  • Licensed in good standing by the State Pharmacy Board in the State in which the hospital is located
  • Successful completion of a pharmacy degree from a U.S.-accredited program or from an international program evaluated by a recognized certifying agency as substantially equivalent to U.S. accreditation standards
  • Minimum 1 year of supervisory experience
  • Proficiency with Microsoft Word and Excel
  • Ability to use remote connectivity to access clinical systems
  • Ability to use Powerchart and PharmNet to enter, verify, and manage medication orders
  • Ability to research and respond to drug information questions using appropriate online resources
  • Knowledge of hospital policies, protocols, procedures, formulary and non-formulary medications
  • Ability to calculate pharmacokinetic dosing, creatinine clearance, renal dosing, and medication intervals
  • Ability to manage protected health information consistently with privacy rules
