About the role
Key responsibilities & impact- Assess assigned caseloads of behavioral health care patients and families to identify physical, psychosocial, and environmental needs
- Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
- Assess patient needs and incorporate physical, psychological, social, and spiritual data into plans of care
- Perform initial and follow-up evaluations and re-evaluate patients’ nursing needs during visits
- Communicate significant findings, problems, health-condition changes, environmental concerns, and unsafe facility conditions to the Clinical Supervisor, physician, facility, and other care personnel
- Revise plans of care in response to identified patient-care issues and notify relevant clinical and facility personnel
- Counsel, instruct, and involve patients, facilities, and families in following the Interdisciplinary Plan of Care
- Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings
- Initiate, supervise, and document behavioral health aide and LPN care delivery according to agency policy
- Inform supervisors of unusual or potentially problematic nursing-facility issues
- Provide support at time of death and perform bereavement assessments
- Report to the Clinical Manager or Clinical Supervisor
Requirements
What you’ll need- Current, unrestricted state RN license
- 1+ years of experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must be able and willing to travel 50%
- Associate Degree or Bachelor’s Degree in Nursing is preferred
- Previous Home Health or Hospice experience is preferred
- Previous experience with Homecare HomeBase is preferred
