About the role
Key responsibilities & impact- Provide home health CNA services to patients in the comfort of their homes
- Support patients with medical needs and help them remain safe and comfortable at home
- Work closely with a skilled interdisciplinary team
- Collaborate with Registered Nurses, LPNs, Physical Therapists, Occupational Therapists, Speech Therapists, and other interdisciplinary team members
- Provide services throughout the Shelton Branch coverage area
- Assist with patient transfers and move equipment
Requirements
What you’ll need- Must be certified and active with the CT Nurse Aide Registry
- Current Certified Nursing Assistant (CNA) license in the state of employment
- Compassionate, patient-focused approach to care
- Excellent communication and interpersonal skills
- Ability to lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment
- Reliable transportation
- Ability and willingness to travel throughout the full Shelton Branch service area
