About the role

Key responsibilities & impact
  • Provide hands-on personal care under the direction of a Registered Nurse
  • Assist patients with bathing, dressing, grooming, toileting, and other daily activities
  • Take and record vital signs, including temperature, pulse, respirations, and blood pressure
  • Help patients safely walk, transfer, and use mobility equipment
  • Follow each patient's individualized care plan
  • Monitor and report changes in a patient's condition to the care team
  • Help maintain a clean, safe, and comfortable home environment
  • Remind patients to take medications as directed
  • Respond appropriately to emergency situations and follow safety and infection control guidelines
  • Maintain accurate documentation, protect patient privacy, and participate in required training

Requirements

What you’ll need
  • Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
