About the role

Key responsibilities & impact
  • Promote Elara Caring’s philosophy, mission statement, and administrative policies to ensure quality of care
  • Assist in implementing rehabilitation needs by following a plan of care based on Occupational Therapist findings
  • Make routine visits and re-evaluate patients’ therapy needs during each visit
  • Communicate significant findings, problems, and changes in condition or environment to supervisors, Occupational Therapist, physician, and other care team members
  • Verify the Plan of Care before each visit and provide care according to physician’s orders, assessment data, standards, and guidelines
  • Counsel and instruct patients and caregivers on therapy needs and home exercise programs
  • Promote compliance with fiscal intermediary and third-party payer requirements through education, coaching, and assistance
  • Identify and report potential payment or coverage problems
  • Maintain compliance with federal, state, and local regulations and Medicare, Medicaid, and reimbursement regulations
  • Assure continuity and quality of patient care
  • Maintain communication with clinical staff regarding schedules, patient data, needs, physician orders, and special instructions
  • Enter clinical notes in a timely manner and coordinate services with physicians, staff, and disciplines
  • Implement actions to prevent avoidable hospitalizations and emergency-room visits
  • Report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee

Requirements

What you’ll need
  • Graduate of a certified occupational therapy curriculum established by the Occupational Therapy Association
  • Current, unrestricted COTA certification in the state of work
  • Minimum one (1) year of experience as a COTA in a clinical care setting
  • Able and willing to travel within branch/office coverage area
  • Dependable vehicle
  • Valid driver’s license
  • Current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Ability to work in a physically demanding, high-stress environment
  • Ability to work irregular hours, including call hours if applicable
