About the role
Key responsibilities & impact- Complete start-of-care visits and comprehensive assessments
- Evaluate clinical needs, medications, home safety, and caregiver support
- Create individualized plans of care with physicians and the clinical team
- Educate patients and families about the care plan
- Document admissions accurately
- Communicate key findings to the care team
- Conduct admissions visits across Newburyport to Haverhill, Greater Boston, and the North Shore
- Use independent clinical judgment to transition patients into ongoing care
Requirements
What you’ll need- Active, unrestricted Massachusetts RN license
- At least one year of nursing experience
- Home health experience is required
- Admissions or start-of-care experience is preferred
- Strong assessment, documentation, and independent decision-making skills
- Valid driver’s license
- Reliable transportation
- Current auto insurance
- Experience with HomeCare HomeBase (HCHB) is a plus
- Associate degree in nursing is required
- BSN is preferred
