About the role
Key responsibilities & impact- Provide oversight of all patient care services
- Coordinate patient care and referrals
- Continually assess patient needs
- Develop, implement, and update individualized patient plans of care
- Review and approve plans of care and evaluate proposed changes for clinical appropriateness
- Conduct regular OASIS reviews for all patients
- Ensure required visits are completed timely
- Review plans of care and paperwork for accuracy and compliance with State and Federal requirements
- Monitor timeliness of required documentation uploaded to HCHB by direct care team members and LPN/LVN Clinical Coordinators
- Maintain records and required documentation, ensuring accuracy, completeness, and compliance with licensing regulations, certification standards, and legal and ethical requirements
- Participate in strategic planning and QAPI activities
- Perform other assigned duties and projects
Requirements
What you’ll need- Associate degree in a Nursing related field required
- 2 years home care experience as a registered nurse in a Home Health
- Current, unrestricted RN license for Massachusetts
- Ability to quickly become proficient in enterprise applications such as Homecare Homebase and Workday
- Knowledge of Microsoft Office
- May be asked to travel on occasion to support a field office in time of audit or necessitating event
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Required to attend branch staff meetings quarterly
