About the role

Key responsibilities & impact
  • Provide skilled nursing care according to each patient’s individualized care plan and physician orders
  • Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager
  • Teach patients and caregivers techniques that support health, safety, and independence at home
  • Collaborate with the RN Case Manager as patient needs and care plans change
  • Document visits, nursing interventions, and medication information accurately and promptly
  • Communicate with physicians, patients, families, and the clinical team to keep care coordinated
  • Help reduce avoidable hospitalizations through attentive care and early recognition of concerns

Requirements

What you’ll need
  • An active Massachusetts LPN license
  • Graduation from an accredited nursing program
  • At least one year of clinical nursing experience
  • A valid driver’s license, reliable transportation, and current auto insurance
  • Ability to travel between patient homes and assigned work locations
  • Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
