About the role
Key responsibilities & impact- Provide nursing care in patients’ homes according to their plan of care
- Monitor patients and share changes with the RN Case Manager
- Teach patients and caregivers how to manage care at home
- Document visits, interventions, and medication updates
- Work with the care team to help patients avoid unnecessary hospital visits
- Provide skilled, personal care to patients in the Palmer and Monson area
- Work alongside an RN Case Manager and supportive clinical team
Requirements
What you’ll need- An active Massachusetts LPN license
- Graduation from an accredited nursing program
- At least one year of clinical nursing experience
- A valid driver’s license, reliable transportation, and current auto insurance
- Ability to travel throughout the Palmer and Monson territory
- Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
