About the role
Key responsibilities & impact- Promote Elara Caring’s philosophy, mission statement, and administrative policies to ensure quality of care
- Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists
- Review patients’ functional needs and adjust care plans to ensure optimal outcomes
- Design and implement patient care plans based on thorough assessments
- Incorporate participation by significant others when appropriate
- Conduct therapy practice within defined standards of care
- Direct support staff to achieve patients’ functional goals
- Document evaluations, treatment goals, plans, prescriptions, and signatures
- Regularly update and maintain documentation
- Provide direction and leadership to staff
- Act as a resource to promote physical therapy in the community
- Work in a physically demanding, high-stress environment, including handling, lifting, and transferring patients
- Potentially work irregular hours, including call hours if applicable
- Report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee
Requirements
What you’ll need- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Able and willing to travel within branch/office coverage area
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
