About the role
Key responsibilities & impact- Follow each patient's individualized care plan and physician orders
- Monitor patient progress and report changes or safety concerns to the care team
- Help patients and caregivers learn self-care techniques to support their health
- Work with the RN Case Manager to update care plans as patient needs change
- Document medication regimens and update medication profiles according to agency guidelines
- Communicate with physicians, patients, families, and the care team to ensure coordinated care
- Help prevent avoidable hospitalizations and emergency room visits through proactive patient care
- Deliver compassionate, high-quality care that supports positive patient outcomes
Requirements
What you’ll need- Current State License as a Licensed Vocational / Practical Nurse
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- 1 year of experience in a clinical care setting
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
- Experience in a hospice or home health environment is preferred
