About the role

Key responsibilities & impact
  • Support home health patients and families throughout Portland and surrounding southern Maine communities
  • Assess patients’ psychosocial, financial, and environmental needs

Requirements

What you’ll need
  • Master’s or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education
  • Current Maine Social Worker license in good standing
  • At least one year of social work experience in a healthcare setting
  • Strong communication, counseling, documentation, and advocacy skills
  • Ability to work independently in patients’ homes
  • Willingness to travel throughout the assigned service area with 50% travel required
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
