About the role
Key responsibilities & impact- Assess patients to identify psychosocial, financial, environmental, and safety needs
- Complete initial social work evaluations and reassess patient needs during follow-up visits
- Develop and update individualized care plans based on patient needs, goals, and physician orders
- Communicate changes in patient condition, concerns, or safety issues to physicians and the care team
- Connect patients and families with appropriate community resources and support services
- Counsel and educate patients and families to help address social, emotional, and care-related needs
- Coordinate with the care team to support continuity of care and effective discharge planning
- Provide compassionate, high-quality social work services that support positive patient outcomes
Requirements
What you’ll need- Current State License as a Social Worker
- Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
- 1+ year of social work experience in a healthcare setting
- Social work skills as defined as general social work standards
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
