About the role
Key responsibilities & impact- Evaluate occupational therapy patients to determine rehabilitation needs and potential for achievement
- Develop and implement individualized Plans of Care
- Perform initial therapy evaluations and re-evaluate patients’ therapy needs during visits
- Conduct follow-up assessments according to OASIS and PPS regulatory guidelines
- Communicate significant findings, problems, and changes in patient condition or environment to supervisors, physicians, and care team members
- Initiate patient safety plans using patient, family, and community resources
- Verify Plans of Care and provide care according to physician orders, assessment data, standards, and guidelines
- Write physician orders for additional visits and Plan of Care changes according to agency policy
- Document therapy care goals and services in therapy notes
- Provide occupational therapy services according to accepted standards of practice and physician certification
- Instruct patients and families in activities of daily living, adaptive equipment, sensory integration, and fine motor coordination
- Assist patients and caregivers in obtaining necessary equipment, including fabrication of custom-made splints
- Instruct patients and caregivers in home therapy programs and provide modifications or clarification
- Counsel and instruct patients and families regarding therapy and related needs
- Supervise or provide clinical direction to COTAs, HHAs, and clinical staff
- Report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee
Requirements
What you’ll need- Graduate of an accredited Occupational Therapy program
- Current, unrestricted OT license valid for the state of work
- Minimum one (1) year of experience in a clinical care setting
- Able and willing to travel within branch/office coverage area
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Ability to work in a physically demanding, high-stress environment
- Full range of body motion, including handling, lifting, and transferring patients
- Potentially work irregular hours, including call hours if applicable
