About the role

Key responsibilities & impact
  • Evaluate patients’ mobility, strength, balance, functional abilities, and home safety needs
  • Develop and implement individualized Plans of Care
  • Provide hands-on therapy to improve mobility, independence, and quality of life
  • Establish measurable treatment goals and monitor patient progress
  • Adjust treatment plans based on changes in condition and functional outcomes
  • Educate patients, caregivers, and family members on exercises, equipment, and safety
  • Coordinate care with physicians, nurses, therapists, and other care team members
  • Direct and support therapy assistants when applicable
  • Complete accurate and timely evaluations, treatment notes, and clinical documentation
  • Serve as a clinical resource and advocate for patients receiving therapy at home

Requirements

What you’ll need
  • Graduate of an American Physical Therapy Association-approved physical therapy program
  • Current and unrestricted Connecticut Physical Therapist license
  • At least one year of experience in a healthcare setting
  • Strong assessment, communication, organization, and documentation skills
  • Ability to work independently in patients’ homes
  • Willingness to travel throughout Stamford and the assigned coverage area
  • Must be able and willing to travel 50%
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
