About the role
Key responsibilities & impact- Review patient records prior to admission to ensure adequate clinical documentation and critical information required for billing and care delivery
- Manage an area's pending admissions, including patients awaiting discharge or pending surgery
- Coordinate between sales and the referring facility
- Ensure the patient is informed and converts
- Support the team of compassionate and dedicated healthcare providers
- Report to the Manager of Pre Admission Coordinators
Requirements
What you’ll need- Degree from an accredited school of professional nursing
- Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
- 1 year of practical experience
- Home Health or Hospice experience is preferred
- Ability to convey information and opinions, listen to others, check for understanding, and respond in an appropriate message and tone
- Ability to set timeframes, match resources to tasks, and plan ahead
- Ability to make decisions with incomplete information, choose from multiple options, and know when to escalate
- Ability to quickly become proficient in enterprise applications such as Homecare Homebase and Workday
- Intermediate knowledge of Microsoft Office
- Ability to handle complaints, manage conflict, and negotiate outcomes
- Ability to seek patient feedback, advocate for patients, understand patient needs, and manage patient expectations
- Bilingual in English and Spanish is preferred
- Valid driver's license and insurance
- Reliable transportation to perform job tasks
