About the role
Key responsibilities & impact- Work Friday-Sunday or Saturday-Monday on 12-hour shifts in the Warsaw area
- Promote Elara Caring’s philosophy, mission statement, and administrative policies to ensure quality of care
- Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
- Respond to daily patient assignments according to divisional guidelines
- Submit all documentation and receive physician orders
- Review patient assignment data, including the Plan of Care and Coordination Notes, before service delivery
- Use clinical assessment skills to make sound judgments and decisions regarding health issues
- Adapt day-to-day scheduling to client needs
- Report and document Client Complaint and Client Occurrence Events
- Observe clients for abnormal signs and symptoms and report them immediately to the physician, RN Case Manager, Clinical Team Manager, and/or Director
- Report significant patient findings and collaborate on actions or interventions
- Document services provided and changes in client condition or caregiver or home situation
- Submit clinical notes in a timely manner
- Maintain communication with branch leadership, RN Case Manager, and Scheduler regarding schedule changes, patient data or needs, physician orders, and special instructions
- Participate in continual review and evaluation of clinical practices and learning activities
- Perform other assigned duties and projects
Requirements
What you’ll need- Graduate of an accredited school of nursing
- 1+ year of experience as a nurse in a clinical setting
- Registered Nurse (RN) in the state or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
- Must be able to travel to patient and client homes with a dependable vehicle
- Driver's license and insurance maintained as required by state law
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
