About the role
Key responsibilities & impact- Conduct start-of-care visits, supervisory visits, and client assessments
- Develop, review, and revise individualized plans of care based on patient needs
- Supervise homemakers, personal care aides, and home health aides
- Monitor and report significant changes in patients’ physical and mental conditions
- Educate caregivers, patients, and families using effective teaching and learning principles
- Ensure all services meet State, Federal, contract, and licensure requirements
- Conduct caregiver orientations and performance assessments
- Evaluate home environments for safety risks and recommend solutions
- Maintain accurate clinical documentation, care plans, and service records
- Communicate with caregivers, clients, case managers, and leadership teams
- Participate in staff meetings, in-services, and continuing education opportunities
- Support Elara Caring’s mission, values, and patient-centered care
- Report to the Director of Nursing or Clinical Manager
Requirements
What you’ll need- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license in good standing in the state applying for
- CPR Certification required
- Minimum of one (1) year of nursing experience
- Knowledge of Medicare/Medicaid, home care benefits, and applicable State regulations preferred
- Strong clinical assessment, communication, and organizational skills
- Excellent computer proficiency and documentation abilities
- Ability to multitask and adapt in a fast-paced healthcare environment
- Professional verbal and written communication skills
- Ability to motivate and lead caregivers in a positive and supportive manner
- Reliable personal transportation is required
- Ability to travel locally and regionally to support additional field offices as needed
