About the role
Key responsibilities & impact- Visit patients in their homes to assess their health and care needs
- Complete initial, ongoing, and focused nursing assessments
- Develop and update plans of care with patients, families, physicians, and the care team
- Provide nursing care according to physician orders and the patient’s plan of care
- Monitor patient conditions, recognize changes, and communicate concerns to the care team
- Teach patients and caregivers about medications, treatment plans, and home safety
- Document visits and care accurately and promptly in Homecare Homebase (HCHB)
- Help ensure each patient receives safe, coordinated, and compassionate care
Requirements
What you’ll need- Current Registered Nurse license in the state of practice
- Graduation from an accredited nursing program
- At least one year of nursing experience in a clinical setting
- Home health experience
- Familiarity with Homecare Homebase (HCHB)
- Valid driver’s license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations
- Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment
