About the role

Key responsibilities & impact
  • Assess assigned caseloads of behavioral health care patients and families to identify physical, psychosocial, and environmental needs
  • Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
  • Assess patient needs and incorporate physical, psychological, social, and spiritual factors into plans of care
  • Perform initial evaluations and reevaluate patients’ nursing needs during each visit
  • Make follow-up visits according to patient, family, and facility needs
  • Communicate significant findings, problems, health-condition changes, environmental issues, and unsafe facility conditions to relevant clinical personnel
  • Revise plans of care in response to identified patient-care issues and notify relevant team members
  • Counsel and instruct patients, facilities, and families regarding the Interdisciplinary Plan of Care
  • Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings
  • Initiate, supervise, and document behavioral health aide and LPN care delivery according to agency policy
  • Inform supervisors of unusual or potentially problematic nursing-facility issues
  • Provide appropriate support at time of death and perform bereavement assessments
  • Report to the Clinical Manager or Clinical Supervisor

Requirements

What you’ll need
  • Current, unrestricted state RN licensee
  • 1+ years of experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Dependable vehicle
  • Valid driver’s license
  • Current auto insurance in accordance with state laws
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must be able and willing to travel 50%
  • Associates Degree or Bachelors Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with Homecare HomeBase is preferred
