About the role

Key responsibilities & impact
  • Assess individual clients’ health needs and determine whether home care admission is appropriate
  • Interview clients and/or caregivers to determine expectations and needs
  • Complete admission assessments and develop personalized plans of care
  • Coordinate with referral sources to support smooth transitions to home care
  • Develop plans of care for home health aides, RNs, and LPNs
  • Communicate with clients, caregivers, and physicians to facilitate continuity of care
  • Identify goals and provide teaching materials and community resource information
  • Coordinate equipment, supplies, pharmacy, medical services, transportation, and other services
  • Implement and update plans of care based on assessments and supervisory visits
  • Evaluate plan-of-care effectiveness at recertification and as needed
  • Supervise private duty RNs, LPNs, and home health aides
  • Review supervised disciplines’ documentation for appropriateness and completion
  • Report to the Branch Director or Clinical Manager

Requirements

What you’ll need
  • Graduate of an accredited RN Program
  • Current state license as RN
  • 2+ years’ experience in a clinical care setting or home health environment preferred
  • Excellent communication skills
  • Dedication to patient care
  • Ability to work as part of inter-disciplinary team
