About the role
Key responsibilities & impact- Assess patients to determine their care needs
- Complete initial and ongoing patient assessments
- Protect patient and staff privacy in accordance with HIPAA
- Provide nursing care according to the plan of care and physician orders
- Monitor patient conditions and report changes to the care team
- Educate patients and families on their care and safety
- Take appropriate action when patient conditions change
- Help ensure quality, safe, and continuous patient care
- Serve patients in Ashtabula, Lake, and Geauga Counties
Requirements
What you’ll need- Current State license as a Registered Nurse
- Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing
- 1+ year of experience as a nurse in a clinical care setting
- Home Health experience is preferred, but not required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
