About the role

Key responsibilities & impact
  • Provide high-quality home health care to patients in their preferred home setting
  • Align activities with Elara Caring’s board, executive team, and Home Health leadership vision
  • Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
  • Assess assigned caseloads of home health care patients and families
  • Identify physical, psychosocial, and environmental patient needs
  • Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
  • Assess physical, psychological, social, and spiritual factors influencing patient, family, and caregiver health status
  • Incorporate assessment data into plans of care
  • Perform initial evaluations and reevaluate patients’ nursing needs during each visit
  • Make follow-up visits according to patient, family, and facility needs
  • Communicate significant findings, problems, and health-condition or environmental changes to the Clinical Supervisor, physician, facility, and other care personnel
  • Revise plans of care in response to patient-care issues and notify relevant clinical and care-team members
  • Report to the Clinical Manager or Clinical Supervisor

Requirements

What you’ll need
  • Current, unrestricted state RN licensee
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Dependable vehicle
  • Valid driver’s license
  • Current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Associates Degree or Bachelor’s Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with HomeCare HomeBase is preferred
