About the role

Key responsibilities & impact
  • Provide high-quality home health care to patients in their preferred home settings
  • Align all activities with Elara Caring’s vision and the leadership of the Home Health team
  • Assess assigned caseloads to identify patients’ physical, psychosocial, and environmental needs
  • Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
  • Assess physical, psychological, social, and spiritual factors influencing patient, family, and caregiver health status
  • Conduct initial evaluations and re-evaluate patients’ nursing needs during each visit
  • Make follow-up visits according to patient, family, and facility needs
  • Communicate significant findings, problems, health-condition changes, environmental issues, and unsafe facility conditions to the Clinical Supervisor, physician, facility, and other care personnel
  • Revise plans of care in response to identified patient-care issues and notify the Clinical Supervisor, physician, facility, and other team members
  • Report to the Clinical Manager or Clinical Supervisor

Requirements

What you’ll need
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Associate’s Degree or Bachelor’s Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with HomeCare HomeBase is preferred
  • Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
