About the role

Key responsibilities & impact
  • Manage an assigned caseload of home health patients
  • Complete initial evaluations and ongoing nursing assessments
  • Develop, coordinate, implement, and update individualized Plans of Care
  • Monitor patients’ physical, psychosocial, environmental, and safety needs
  • Provide skilled nursing treatments and patient education
  • Educate caregivers and family members on patients’ care needs
  • Communicate changes in condition to physicians, clinical leadership, and other care team members
  • Coordinate care with nurses, therapists, social workers, physicians, and community providers
  • Complete accurate and timely clinical documentation
  • Maintain patient privacy and confidentiality
  • Support safe transitions and continuity of care

Requirements

What you’ll need
  • Current and unrestricted Registered Nurse license in Maine
  • At least one year of clinical nursing experience
  • Strong assessment, communication, organization, and documentation skills
  • Ability to manage a caseload and work independently in patients’ homes
  • Willingness to travel throughout Wells, York, and the assigned coverage area
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
  • Must be able and willing to travel 50%
  • Associate Degree or Bachelor’s Degree in Nursing preferred
  • Previous home health or hospice experience preferred
  • HomeCare HomeBase experience preferred
  • E-Verify and Form I-9 completion after a job offer is accepted
