About the role

Key responsibilities & impact
  • Assess patients and develop individualized care plans
  • Provide skilled nursing care in patients’ homes
  • Teach patients and caregivers about medications, treatments, and managing their health
  • Coordinate care with physicians and the interdisciplinary team
  • Complete timely, accurate clinical documentation
  • Care for patients in Youngstown, Warren, Cortland, Poland, Boardman, and surrounding communities in Trumbull and Mahoning counties

Requirements

What you’ll need
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations
  • Must be able and willing to travel 50%
  • Associate Degree or Bachelor’s Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with HomeCare HomeBase is preferred
