About the role
Key responsibilities & impact- Promote Elara Caring’s philosophy, mission statement, and administrative policies to ensure quality of care
- Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
- Communicate significant findings, problems, and changes in condition or environment to the Supervisor, physician, and/or other personnel involved with patient care
- Report unsafe conditions and the outcome of each visit to the appropriate Supervisor by the end of the day
- Implement the plan for patient safety using patient, family, and community resources
- Participate in implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning
- Verify the Plan of Care prior to each visit and provide care according to physician’s orders, assessment data, and established standards and guidelines
- Initiate and revise the Plan of Care in response to identified patient care issues
- Write physician orders to cover additional visits and changes to the plan of care, per agency policy
- Incorporate patient care goals established in the plan of care, as evidenced by documentation in clinical notes
- Perform appropriate skilled services/interventions in accordance with accepted standards of practice and certified by the patient’s physician
- Counsel, instruct, and include patients and families in following the Plan of Care and meeting social work-related needs
- Report to the Clinical Team Manager
Requirements
What you’ll need- Baccalaureate degree in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other fields related to social work (SWA)
- Licensure subject to state requirements
- 1 year of social work experience in a healthcare setting
- Social work skills as defined as general social work standards
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver’s license and insurance
- Reliable transportation to perform job tasks
