  • Develop and implement speech-language therapy plans of care
  • Admit patients and assess speech-language therapy and other needs according to OASIS and PPS regulatory guidelines
  • Perform initial therapy evaluations and re-evaluate patients during visits
  • Conduct follow-up assessments according to OASIS and PPS regulatory guidelines
  • Initiate patient safety plans using patient, family, and community resources
  • Communicate significant findings, problems, and changes in condition or environment to supervisors, physicians, and care team members
  • Participate in development and implementation of plans of care, continuity of care, and discharge planning
  • Verify plans of care and provide care according to physician orders, assessment data, standards, and guidelines
  • Write physician orders for additional visits and plan-of-care changes according to agency policy
  • Document therapy goals and revise plans of care in response to patient care issues
  • Perform direct speech-language pathology services in accordance with accepted standards and physician certification
  • Select diagnostic and therapeutic materials appropriate to patient needs
  • Instruct patients and caregivers in home therapy programs and provide modifications or clarification
  • Maintain communication and coordination with physicians, staff, and other disciplines through clinical notes
  • Ensure therapy notes, supply logs, and visit logs are accurate and submitted according to office policy
  • Report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee

Requirements

What you’ll need
  • Master’s degree from an accredited educational program in Language Pathology
  • Current, unrestricted SLP license in the state of work
  • Minimum one (1) year of experience as a SLP in a clinical care setting
  • Able and willing to travel within branch/office coverage area
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Dependable vehicle
  • Valid driver’s license
  • Current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Full range of body motion, including handling, lifting, and transferring patients
  • Ability to work in a physically demanding, high-stress environment
  • Potentially work irregular hours, including call hours if applicable
