About the role
Key responsibilities & impact- Promote Elara Caring’s philosophy, mission statement, and administrative policies to ensure quality of care
- Evaluate patients to determine rehabilitation needs and potential for achievement
- Develop plans of care based on evaluation findings
- Admit patients and assess speech-language therapy and other needs according to OASIS and PPS regulatory guidelines
- Conduct initial therapy evaluations and re-evaluate speech-language therapy needs during visits
- Complete follow-up assessments according to OASIS and PPS regulatory guidelines
- Initiate patient safety plans using patient, family, and community resources
- Communicate significant findings, problems, and changes in condition or environment to supervisors, physicians, and care team members
- Participate in implementing and developing Plans of Care, continuity of care, and discharge planning
- Verify Plans of Care before visits and provide care according to physician orders, assessment data, and standards
- Write physician orders for additional visits and Plan of Care changes according to agency policy
- Incorporate patient goals into therapy and document them in therapy notes
- Initiate and revise Plans of Care in response to identified patient care issues
- Provide direct speech-language pathology services according to accepted standards and physician certification
- Select diagnostic and therapeutic materials appropriate to patient needs
- Instruct patients and caregivers in home therapy programs and provide modifications or clarification
- Maintain communication and coordination with physicians, staff, and other disciplines through clinical notes
- Ensure therapy notes and supply/visit logs are accurate and submitted according to office policy
- Report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee
Requirements
What you’ll need- Master’s degree from an accredited educational program in Language Pathology
- Current, unrestricted SLP license in the state of work
- Minimum one (1) year of experience as a SLP in a clinical care setting
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Full range of body motion, including handling, lifting, and transferring patients
- Ability to work in a physically demanding, high-stress environment
- Potentially work irregular hours, including call hours if applicable
