About the role
Key responsibilities & impact- Perform case management telephonically and/or through home visits within scope of licensure
- Use screening criteria knowledge and clinical judgment to assess member needs
- Conduct assessments to identify individual needs
- Develop care plans addressing assessment objectives and goals
- Monitor and evaluate care-plan effectiveness and modify plans as needed
- Support member access to appropriate, quality, and cost-effective care
- Coordinate with internal and external resources to meet identified member needs
- Collaborate with providers
Requirements
What you’ll need- Must be within the state of Virginia; Norfolk and Richmond, VA preferred
- MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or equivalent combination of education and experience
- Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist license, as applicable by state law and scope of practice, required
- Experience in case management and telephonic and/or in-person coaching with members with complex psychiatric, substance abuse, and/or medical disorders preferred
- For Government business, LAPC or LAMFT may also be acceptable as allowed by applicable state laws
- For associates working within Puerto Rico in specified member- or patient-facing roles, current PR health certificate and current PR Law 300 certificate are required
- Must be able to work Monday–Friday, 8:00am–5:00pm EST
- Candidates must be within reasonable commuting distance from posting location(s) unless an accommodation is granted
- Certain patient/member-facing roles require COVID-19 and Influenza vaccination, subject to stated exceptions
